PHOTO RELEASE *

I hereby grant Mental Health Fest, Inc., and any of its volunteers or designees, the irrevocable and exclusive right to use my name, likeness, voice, and/or reputation in all forms and media, whether now known or developed in the future. This includes, but is not limited to, promotional and educational materials. Further, I waive any right to inspect or approve the use, publication, or distribution of any photographs, recordings, or other materials in which I appear. Furthermore, I release and agree to hold harmless Mental Health Fest, Inc., or any of its volunteers or designees, from any liability related to such use of my name, likeness, voice, and/or reputation. I waive any claims arising from such use, including but not limited to invasion of privacy, violation of publicity rights, or defamation. I am over the age of 18, competent to enter into this agreement, and that my execution of this agreement does not and will not violate any other contract to which I am a party. I have read and fully understand this release, and I agree that it will be binding upon me, my heirs, legal representatives, and assigns.

LIABILITY WAIVER *

On behalf of myself, my business/organization, or any associated affiliates, I hereby release and discharge Mental Health Fest, Inc., and any of its affiliates, from any responsibility for loss, damages, injuries, or death in connection with my participation in any Mental Health Fest, Inc. activities. I agree to indemnify and hold harmless Mental Health Fest, Inc., and any of its affiliates from any costs, expenses, or liabilities (including attorney fees) arising from any loss, damage, injury, or death resulting from my participation in any Mental Health Fest, Inc. activities. I acknowledge that Mental Health Fest, Inc., or any of its affiliates do not provide medical insurance coverage for myself, my business/organization, or any associated affiliates. I understand and accept full responsibility for any medical expenses that may arise from my participation or the participation of my business/organization, or any associated affiliates in any Mental Health Fest, Inc. activities.
